Canon Professional Services (CPS) typically will send only one repair form in the
annual packet. This is good if you baby your gear, but for the rest of us
who are constantly cramming stuff in to overhead luggage bins and
the day to day wear it sustains while running around the
sidelines we usually end up with more than one broken item per year.
You don't necessarily need this but I often see it requested in forums and such so I
thought I'd just put a PDF of it online for all to use. REMEMBER, you will only
receive PRO RUSH SERVICE if you are a CPS member.
